I can't really say. In the very small industry that I was involved in, which wasn't even paid, we would face serious backlash for this. It may have been deadlines but magazines back in the day came out once a month, maybe something tiny would come out weekly. Newspapers were daily but nobody can tell me that a highly paid editor can't see these things. They are quite obvious. My guess would be that they just didn't care since most of these mistakes were happening towards the end of print days.
