Normalmente no me presento como la doctora, sino como Anakarina Marcano, dependiendo del contexto agrego frases para romper el hielo como "Lo más bello de..." y se genera una reacción muy linda en las personas que me causa gracia, a pesar de tener una carrera maravillosa, no me gusta andar haciendo alarde de la misma, porque pienso que todos en el mundo somos igual de necesarios e importantes.

Ayer, estuve de guardia, una vez más, ya que estoy haciendo Postgrado para especializarme como Anestesiólogo. He tenido muchas experiencias realmente satisfactorias en esta etapa y hoy decidí compartir un día de mi vida profesional con ustedes. Actualmente soy residente de tercer año de Postgrado, normalmente nos autodenominamos "R3", la letra significa Residente y el número hace alusión al año.
Yesterday, I was on call, once again, as I am doing Postgraduate studies to specialize as an Anesthesiologist. I have had many really satisfying experiences at this stage and today I decided to share a day of my professional life with you. I am currently a third year Postgraduate resident, we usually call ourselves "R3", the letter stands for Resident and the number refers to the year.
Lo primero que ocurre en mi día de guardia es el sonido de la alarma, que normalmente no suelo escuchar, no porque me quede dormida, sino todo lo contrario, siento mucha ansiedad por vivir ese día de guardia, saber qué retos se pueden presentar y si seré capaz de realmente ayudar a la gente a resolver esos problemas que los llevan a una mesa quirúrgica. Entonces me pasa que me despierto media hora antes de la hora pautada, luego por media hora tengo una lucha conmigo misma para dormir un ratito más, porque "nunca se sabe cómo será la guardia".
The first thing that happens on my on-call day is the sound of the alarm, which I usually do not hear, not because I oversleep, but on the contrary, I feel very anxious to live that on-call day, to know what challenges may arise and if I will be able to really help people to solve those problems that lead them to a surgical table. Es una tragedia porque siempre queda la sensación de haber dejado "algo necesario" en la casa. Me bañé y arreglé en menos de 10 minutos, preparé mis comidas el día anterior, tenía mis "Panquecas de Auyama" para desayuno y la cena. Llene dos envases de 2 litros con agua filtrada y luego de ponerme mi "disfraz de médico" llamé a mi papá para avisarle que estaba lista, y en un santiamén ya estaba pasando por mi para llevarme al hospital.

Una vez que recibí mi guardia me coloqué el "EPP" (Equipo de Protección Personal), que consiste en mono quirúrgico, bata quirúrgica, tapabocas, guantes y careta (que por cierto produce mucho dolor de cabeza) y salí al ruedo... Por cierto, si quieres identificar rápidamente a la gente de Anestesiología, somos los únicos con un estetoscopio en quirófano 😆
Once I received my guard I put on my "PPE" (Personal Protective Equipment), which consists of surgical overalls, surgical gown, face shield, gloves and face shield (which by the way causes a lot of headache) and I went out into the ring..... Como rutina, nuestra prioridad siempre son los pacientes, si recibimos algún paciente crítico, mantener a este paciente estable es nuestro objetivo principal. Y ese fue el caso de la guardia, tenía un paciente crítico en el área de recuperación post Anestesia, había sido intervenido por neurocirugía y requería cupo en UCI (Unidad de Cuidados Intensivos) pero no había cupo, así que estaba con nosotros, pero para nuestra alegría, su servicio tratante estaba muy al pendiente de ella, y ella tenía, a pesar de todo, buen pronóstico.

Dentro de todo el estrés, siempre tenemos algo como objetivo principal, mantenernos sanos y a salvo. Entonces el uso correcto del EPP y las horas de descanso y comida son nuestra prioridad, para eso nos relevamos e hicimos turnos para todos poder comer a tiempo.
Metí una paciente de Ginecología, ella en particular me saco una sonrisa, estaba muy asustada y a pesar de ser su cuarto embarazo, era su primer hijo, los demás habían fallecido antes del parto y realmente hasta yo quise llorar cuando escuché el llanto de su primer bebé. Su cara de emoción al conocer a su hijo fue un poema total, por momentos como ese es que agradezco haber elegido este camino y no otro.
Within all the stress, we always have one thing as our main objective, to stay healthy and safe. So the correct use of PPE and rest and meal times are our priority, so we took turns and took shifts so we could all eat on time.
I brought in a patient from Gynecology, she in particular brought a smile to my face, she was very scared and even though it was her fourth pregnancy, it was her first child, the others had passed away before delivery and I really wanted to cry when I heard the cry of her first baby. Her face of emotion when she met her son was a total poem, it is for moments like that that I am grateful to have chosen this path and not another.
Cuando le di anestesia a la señora le expliqué todo el procedimiento y ella autorizó el mismo, cuando se acostó en la mesa quirúrgica hice lo que llamamos "Barrera Hemato Encefálica", porque es un límite entre la sangre (sitio donde operan) y la cabecera del paciente, dónde curiosamente siempre debemos estar, para brindar todo el apoyo necesario y estar pendiente de las constantes vitales y las necesidades del paciente. Curiosamente en nuestro cuerpo existe esa llamada barrera hematoencefálica, es la que impide que entren sustancias y elementos que circulan en la sangre y que pueden ser dañinos a nuestro cerebro y nosotros a manera de chiste la usamos así jajaja
When I gave anesthesia to the lady I explained the whole procedure to her and she authorized it, when she lay down on the surgical table I did what we call "Hematoencephalic Barrier", because it is a limit between the blood (the place where they operate) and the patient's bedside, where curiously we must always be, to provide all the necessary support and be aware of the vital signs and the patient's needs. Curiously, in our body there is this so-called blood-brain barrier, which prevents the entry of substances and elements that circulate in the blood and can be harmful to our brain and we use it as a joke hahaha.
Nosotros tenemos mucha responsabilidad dentro de quirófano, no solo es el bienestar del paciente, también debemos hacer reporte de todo, por suerte contamos con un formato de historia bastante completo y resumido que permite hacerlo sin restar mucho tiempo.

Un amigo de la Universidad que está haciendo Postgrado de Cirugía me enseñó a hacer un procedimiento que debería ser parte de nuestra práctica pero que normalmente lo hacen ellos, poner una vía central! Que emoción!! tenía desde R1 queriendo hacer eso!
A friend of mine from the University who is doing Postgraduate Surgery taught me how to do a procedure that should be part of our practice but is usually done by them, put a central line! What a thrill!!! I had been wanting to do that since R1!
Lo mejor de esa guardia fue que a pesar del trabajo pudimos descansar, dejar todo en orden para el grupo de guardia del día siguiente y marcharnos con la misma energía que llegamos. Fue un día fenomenal, de aprendizaje, práctica y entendimiento. Parte de la rutina, pero fuera de ella. Lo mas rico es quitarse el gorro y la careta que te estrangula el pensamiento y un segundo el tapabocas que en combinación con mis lentes me ponen a sufrir las orejas 🤭
The best thing about that watch was that despite the work we were able to rest, leave everything in order for the next day's watch group and leave with the same energy as when we arrived. It was a phenomenal day of learning, practice and understanding. Part of the routine, but out of it. The best thing is to take off the cap and the mask that strangles your thoughts and for a second the mask that in combination with my glasses makes my ears suffer 🤭.
Lo mejor de todo es saber que te vas a casa habiendo logrado los objetivos del día, ayudando sin miramientos y haciendo todo lo posible porque esa experiencia tan traumática que es ser operado, sea lo más amena posible.
En definitiva disfrute al máximo mi día de Guardia, si me preguntan, volvería a escoger Anestesiología una y mil veces más!
The best of all is knowing that you go home having achieved the objectives of the day, helping without regard and doing everything possible to make the traumatic experience of being operated on as pleasant as possible.
In short, I enjoyed my day as a Guard to the fullest. If you ask me, I would choose Anesthesiology again and again!
