Hola muy buenas mis amigos, en esta oportunidad les traigo el mantenimiento de un equipo que ha salvado muchas vidas durante la pandemia del COVID-19, algunas personas tienen miedo al escuchar ventilación mecánica o el paciente sera intubado; en este post voy a ir hablando del ventilar mecánico y así poder aclarar algunas de tantas dudas. ¡Recuerdan que mi punto de vista es desde un ingeniero biomédico, de querer una información más directa con respecto a un paciente, por favor consultar a su medico especialista. Así que no esperemos y ¡vamos allá!

Hello my friends, in this opportunity I bring you the maintenance of an equipment that has saved many lives during the pandemic of COVID-19, some people are afraid to hear mechanical ventilation or the patient will be intubated; in this post I will be talking about mechanical ventilation and thus to clarify some of many doubts. Como siempre hago en mis post, voy a ir dando información sobre este equipo, hasta llegar a mostrarles un pequeño mantenimiento rutinario.

Ventilación mecánica

Mechanical ventilation

Es un modo que utiliza un aparato mecánico para el soporte artificial de la ventilación y oxigenación. Esta es su función principal, mantener un volumen de aire (ventilar), con una concentración determinada de oxigeno (FiO2), por una unidad de tiempo (T).

is a mode that uses a mechanical device for the artificial support of ventilation and oxygenation. This is its main function, to maintain a volume of air (ventilation), with a determined concentration of oxygen (FiO2), for a unit of time (T).

Ahora que entendemos el concepto de ventilación mecánica, también debemos entender la fisiología respiratoria.

Now that we understand the concept of mechanical ventilation, we must also understand respiratory physiology.

Evolución, naturaleza o creación divina de un todo poderoso, sea cual sea la teoría en la que creas no podemos negar que el cuerpo humano es muy perfecto y está muy bien hecho, en el caso del sistema respiratorio algo indujo a los pulmones a estar tan cerca del cerebro, parece que está más cerca del cerebro que otros órganos. La respiración normal se produce gracias al intercambio gaseoso y para ello el gas en los pulmones se puede renovar mediante la inspiración y la espiración.

The normal respiration is produced thanks to the gaseous exchange and for this the gas in the lungs can be renewed by means of inspiration and expiration.

A una escala más microscópica, la verdadera respiración es la que tiene lugar entre la célula y la mitocondria, ya que es allí donde se produce el intercambio de energía, es decir, que el trisfosfato de adenosina (ATP) se convierte en disfosfato de adenosina (ADP), este proceso de oxidación requiere oxígeno.

On a more microscopic scale, the real respiration is the one that takes place between the cell and the mitochondria, since it is there where the energy exchange takes place, that is to say that Adenosine Trisphosphate (ATP) is converted into Adenosine Disphosphate (ADP), this oxidation process requires oxygen.

Debemos entender que nuestros pulmones no están llenos de oxígeno al 100%, contienen diferentes cantidades de gases y una pequeña cantidad de agua, durante la inspiración los pulmones están llenos de 20,48% de O2, 0,04% de CO2, 78,8% de N2 y 0,5% de H2O. Durante la espiración, la concentración de gases que queda en el pulmón es del 15,26% de O2, 3,42% de CO2, 75,13% de N2 y 6,19% de H2O. El N2 es un gas muy ligero que permanece en los pulmones para evitar que se vacíen por completo y se produzca un colapso pulmonar, conocido clínicamente como atelectasia. Por otro lado, la cantidad de agua ayuda a mantener el aire húmedo para que los alvéolos y los tejidos pulmonares no se dañen.

We must understand that our lungs are not filled with 100% oxygen, they contain different amounts of gases and a small amount of water, during inspiration the lungs are filled with 20.48% O2, 0.04% CO2, 78.8% N2 and 0.5% H2O. During expiration the concentration of gases remaining in the lung is 15.26% O2, 3.42% CO2, 75.13% N2 and 6.19% H2O. The N2 is a very light gas that remains in the lungs to prevent them from becoming completely empty and causing a collapse of the lungs, clinically known as atelectasis. On the other hand, the amount of water helps to keep the air humid so that the alveoli and lung tissues are not damaged.

¿Cuándo necesita un paciente ventilación mecánica?

When does a patient require mechanical ventilation?

Un individuo requiere ventilación mecánica cuando tiene insuficiencia respiratoria, cuando no puede controlar el impulso muscular respiratorio, presenta Síndrome de Dificultad Respiratoria Aguda, entre otros cuadros clínicos respiratorios.

An individual requires mechanical ventilation when he/she has respiratory failure, when he/she cannot control the respiratory muscle impulse, presents Acute Respiratory Distress Syndrome, among other respiratory clinical pictures.

Proceso de Intubación:

Intubation Process:

La ventilación mecánica puede aplicarse de muchas maneras, dependiendo del estado clínico del paciente. Por lo general, se introduce un tubo de plástico en la tráquea a través de la nariz o la boca. Si la persona en cuestión necesita ventilación mecánica durante un periodo de tiempo más largo, el tubo puede introducirse directamente en la tráquea a través de una pequeña incisión en la parte delantera del cuello (traqueotomía).

Mechanical ventilation can be applied in many ways, depending on the clinical condition of the patient. Usually, a plastic tube is inserted through the nose or mouth into the trachea. If the person in question needs mechanical ventilation for a longer period of time, the tube can be inserted directly into the trachea through a small incision in the front of the neck (tracheotomy).

El ventilador Artificial es un equipo compacto, casi siempre atractivo, que a la persona que nunca lo ha manejado no suele asustarse al aproximarse, a menos que sea muy curiosa. En la parte posterior del equipo solemos encontrar las conexiones a la toma de alimentación, concretamente la electricidad y las tomas de los gases medicinales. Ademas, hay un interruptor general y puertos para la adquisición de datos, así como un sistema de refrigeración para el equipo.

The Artificial ventilator is a compact, almost always attractive equipment, that the person who has never handled it does not usually get scared when approaching it, unless he/she is very curious. On the back of the equipment we usually find the connections to the power supply, namely the electricity and medical gas connections. In addition, there is a main switch and ports for data acquisition, as well as a cooling system for the equipment.

Si nos adentramos en el equipo, encontraremos tarjetas electrónicas y un sistema neumático, que se encargan de monitorizar, realizar el proceso de ventilación, mezclar los gases adecuadamente, entre otros... Este proceso de mezcla nos permitirá tener siempre una concentración de oxígeno conocida dentro de un rango del 21% al 100% para el gas inspirado. Mediante una válvula proporcional y su mayor o menor apertura, se conseguirá la mezcla deseada, y para que ésta se produzca correctamente, la presión de los gases medicinales debe ser constante y equivalente.

If we go inside the equipment, we will find electronic cards and a pneumatic system, which are in charge of monitoring, performing the ventilation process, mixing the gases properly, among others... This mixing process will always allow us to have a known oxygen concentration within a range of 21% to 100% for the inspired gas. By means of a proportional valve and its greater or lesser opening, the desired mixing will be achieved, and for this to occur properly, the pressure of the medical gases must be constant and equivalent.

En la parte frontal tenemos, la pantalla y una serie de teclas de función fija, así como el mando giratorio, permiten acceder a las páginas de visualización de la aplicación correspondiente. Además de las curvas, la pantalla muestra los valores medidos y las indicaciones de estado en un campo separado de teclas y mandos de ajuste que se activan al tacto. Por otro lado, en la parte frontal también tenemos el puerto de la célula de oxígeno, la válvula espiratoria, el sensor de flujo y los puertos inspiratorio y espiratorio.

On the front side, the display and a series of fixed-function keys as well as the rotary knob are used to call up the display pages for the respective application. In addition to the curves, the display shows measured values and status indications in a separate field of keys and setting knobs that are activated by touch. On the other hand on the front side we also get the oxygen cell port, the expiratory valve, the flow sensor and the inspiratory and expiratory ports.

El circuito del paciente es un sistema de trampas de agua y tubos que se conectan al paciente, ya sea por vía endotraqueal o por traqueotomía, este se divide en dos tubos conectados a una pieza en "Y" cada tubo está destinado a la inspiración y a la espiración respectivamente.

The patient circuit is a system of water traps and tubes that are connected to the patient, either by endotracheal or tracheotomy, this is divided into two tubes connected to a piece in "Y" each tube is intended for inspiration and expiration respectively.

Modos de Ventilación:

Ventilatory Modes:

Los ventiladores tienen diferentes modos para ventilar a un paciente, la selección de estos modos dependerá del estado del paciente, estos modos se clasifican en tres que son:

Ventilators have different modes to ventilate a patient, the selection of these modes will depend on the patient's condition, these modes are classified in three which are:

A. Controlados:

  • Control Volumen (VC): A.El ventilador entrega un volumen de gas programado, independientemente de la presión alcanzada en la vía aérea, con una frecuencia tambien programada. Es un modo controlado por volumen o flujo, iniciado por tiempo, limitado por volumen y ciclado por tiempo. Todos lo ciclos son mandatarios, la única variable dependiente es la Presión en la Vía Aérea. La desventaja de este modo, es el mayor riesgo de barotrauma comparado con PCV.

  • Controlado por Presión (PC): Modo Ventilatorio con limitación de presión, ciclado por tiempo, con todos los ciclos mandatorios. La variable que debemos programar es la presión de la vía aérea (Paw). La característica fundamental es que nunca se va a superar la presión prefijada, asi el volumen corriente (Vt) va a depender del nivel de presión, complianza, resistencia y tiempo inspiratorio. El mayor inconveniente es que no garantiza un volumen tidal (Vt) fijo, así las secreciones, broncoespasmo, etc, van a variar el volumen tidal entregado.

  • Controlado por Volumen regulado por presión (VCRP): se trata de una modalidad ventilatoria que mezcla las características de las otras dos. El parametro que tiende a ser constante es el volumen (corriente o minuto). y el respirador modifica el flujo inspiratorio para conseguir ese volumen con las menores presiones posibles (VCRP, VPA, autoflow...).

  • Ventilación Mandatoria Intermitente Sincronizada(SIMV): permite al paciente realizar respiraciones espontáneas intercaladas entre los ciclos mandatorios del ventilador, la palabra sincronizada hace referencia al período de espera que tiene el ventilador antes de un ciclo mandatorio para sincronizar el esfuerzo inspiratorio del paciente con la insuflación del ventilador.

A. Controlled

  • Volume Control (VC): A. The ventilator delivers a programmed volume of gas, regardless of the pressure reached in the airway, at a programmed frequency. It is a volume or flow controlled mode, time initiated, volume limited and time cycled. All cycles are mandatory, the only dependent variable is the airway pressure. The disadvantage of this mode is the higher risk of barotrauma compared to PCV.

  • Pressure Controlled (PC): Ventilatory mode with pressure limitation, time cycled, with all cycles mandatory. The variable to be programmed is the airway pressure (Paw). The fundamental characteristic is that the preset pressure will never be exceeded, so the tidal volume (Vt) will depend on the pressure level, compliance, resistance and inspiratory time. The main disadvantage is that it does not guarantee a fixed tidal volume (Vt), so secretions, bronchospasm, etc, will vary the delivered tidal volume.

  • Pressure Regulated Volume Controlled Ventilation (PRVC): this is a ventilatory modality that mixes the characteristics of the other two. The parameter that tends to be constant is the volume (tidal or minute) and the ventilator modifies the inspiratory flow to achieve that volume with the lowest possible pressures (PRVC, PPV, autoflow...).

  • Synchronized Intermittent Mandatory Ventilation (SIMV): allows the patient to take spontaneous breaths interspersed between mandatory ventilator cycles, the word synchronized refers to the waiting period the ventilator has before a mandatory cycle to synchronize the patient's inspiratory effort with the ventilator's insufflation.

B. Asistidos

  • Volumen Asistido (VA): Durante este régimen asistido el paciente dispara cada respiración. Si necesita apoyo, el ventilador adaptara automáticamente, respiración a respiración, el nivel de asistencia con presión inspiratoria.

  • Presión Soporte: el ventilar suministras respiraciones manteniendo constante la presión preseleccionada durante toda la inspiración, y con flujo retardante.

B. Assisted

  • Volume Assist (VA): During this assisted regimen the patient triggers each breath. If support is needed, the ventilator will automatically adapt, breath by breath, the level of assistance with inspiratory pressure.

  • Pressure Support: the ventilator delivers breaths while maintaining a constant preset pressure throughout inspiration, and with retardant flow.

C. Combinados

  • SIMV (VC) + PS: durante este modo el ventilador administrara respiraciones mandatorias, tal como se describe en (VC), y respiraciones asistidas disparadas por el paciente, tal como se describe en (PS).

  • SIMV (PC) + PS: en este régimen combinado el ventilador administrará respiraciones mandatorias, tal como se ha descrito en (PC), y respiraciones asistidas disparadas por el paciente, tal como se ha descrito en (PS).

C. Combined

  • SIMV (VC) + PS: during this mode the ventilator will deliver mandatory breaths, as described in (VC), and patient-triggered assisted breaths, as described in (PS).

  • SIMV (PC) + PS: in this combined regimen the ventilator will deliver mandatory breaths as described in (PC) and patient-triggered assisted breaths as described in (PS).

Toda esta información fue lo mas resumida posible para que pudiesen entender un poco el funcionamiento de estos equipos y como mantienen el soporte vital de un paciente, asi tambien entender la importancia que este equipamiento medico representa en las Unidades de Cuidados Intensivos y en las areas de COVID-19.

All this information was summarized as much as possible so that they could understand a little of how this equipment works and how it maintains the life support of a patient, as well as understand the importance that this medical equipment represents in the Intensive Care Units and in the areas of COVID-19.

Mantenimiento preventivo rutinario (lavado de válvula espiratoria, calibración del sensor de flujo y oxigeno).

Routine preventive maintenance (expiratory valve flushing, flow and oxygen sensor calibration).

El equipo que se le realizara el mantenimiento pertenece a la marca AEOMED y su modelo es el VG70 este ventilador es un equipo con buenas prestaciones de servicio y con un sistema digital muy moderno.

The equipment to be maintained belongs to the AEOMED brand and its model is the VG70. This fan is an equipment with good service features and with a very modern digital system.

1. Se procede a quitar la válvula espiratoria del equipo, a desarmar con cuidado sin romper la membrana que posee

1. The expiratory valve is removed from the equipment and carefully disassembled without breaking the membrane.

2. Se sumerge en un agente descontaminate adecuado para estas piezas, se deja en remojo durante 12 min y posteriormente se procede a enjuagar y secar.

2. It is immersed in a decontaminating agent suitable for these parts, left to soak for 12 minutes and then rinsed and dried.

3. Se vuelven a montar las piezas de la valvula espiratoria y se integra nuevamente al equipo.

3. The parts of the expiratory valve are reassembled and integrated back into the equipment.

4. Procedemos a realizar las calibraciones pertinentes, con respecto al sensor de flujo y oxígeno. Esto se logra a través del sistema interno que tiene el equipo, para llegar a esto se debe ir a configuraciones, luego a la ventana de calibración, antes de presionar las teclas para iniciar el proceso se debe colocar la tubería de tal manera que se procese para realizar las calibraciones.

4. We proceed to perform the pertinent calibrations, with respect to the flow and oxygen sensor. This is achieved through the internal system that the equipment has, to get to this you must go to configurations, then to the calibration window, before pressing the keys to start the process you must place the tubing in such a way that it is processed to perform the calibrations.

5. Una vez completadas las calibraciones, se programan los valores estándar y luego se realiza un ciclo del equipo para verificar su correcto funcionamiento.

5. Once the calibrations have been completed, standard values are programmed and then the equipment is cycled to verify its proper operation.

Debo recordar que estos equipos son muy delicados y también costosos, ya que de ellos depende la vida de un ser humano, si no tiene la preparación adecuada por favor no manipule el equipo. El conocimiento que se da aquí es puramente para el conocimiento de la importancia de este equipo.

I must remember that these equipments are very delicate and also expensive, because the life of a human being depends on them, if you do not have the adequate preparation please do not manipulate the equipment.
