Medication Delivery Methodology | Metodología de Entrega de medicinas:
- The Beneficiary comes to our foundation with his/her medical prescription or medical report stating the medication prescribed by the Doctor to him/her or his/her family member.
- A Medical Card is filled out to capture the beneficiary's data.
- A Record of Delivery is filled out which establishes the beneficiary's data, address, identity card, medicines delivered, quantities, signature, telephone number and wet stamp.
- A copy of the beneficiary's receipt is attached to the delivery record to confirm the delivery.
- We proceed to deliver the requested medicines according to availability in our medicine stock
- El Beneficiario acude a nuestra fundación con su receta médica o informe médico en donde se establece el medicamento recetado por el Doctor a él/ella o familiar.
- Se llena una Ficha Médica para captar los datos del beneficiario.
- Se llena un Acta de Entrega en donde se establece los datos del beneficiario, direccion, cédula de identidad, medicamentos entregados, cantidades, firma, número de teléfono y sello húmedo.
- A dicha acta se le anexa copia del récipe del beneficiario para constatar la entrega.
- Se procede a entregar las medicinas solicitadas segun disponibilidad en nuestro banco de medicinas.
All the information contained in this publication will be presented each week to give a balance of the expenses incurred in the different tasks carried out by the foundation according to our DHF Proposal.

Toda la información contenida en esta publicación será presentada cada semana para dar balance de los gastos realizados en las distintas labores que sean llevadas a cabo por la fundación gracias a nuestra propuesta en el sistema DHF.
