Apparently it is a forceful reality, in a city in the United States. They say that it is due to a medicine that they have taken as a drug because it is very cheap to obtain and they sell it to everyone who wants to buy it. What strikes me is that it's a whole city, I can't believe everyone wants to take that drug. It seems something impossible.
