We as humans, more often than not, judge other people based on their actions. Of a truth, actions do speak but the question is "do they speak the truth always?" As much as actions do speak, they may not pass on the true intentions of the heart. This is why, even judgements based on actions can be flawed.

Actions, as well as words, can be beclouded by sentiments and as such, may affect our sense of judgement. I'll explain what I mean by this with the short story of what happened to my friend.
A few days back, my friend went to pick her kids from the school. On heading home, they normally passed through a very sloppy alley. In that fateful day, as she was descending through the steeps of the alley in her car, the brake failed and to worsen the matter, the end of the road culminated in a very busy mini-market.
As her car was descending through the slope, she had a matter of split seconds to effect an on-the-spot decision. The only thing that came to her mind was to swerve to the right and brake her car with the cars parked along the road.
Well, the plan worked and the car was halted before running into the market. However, the car she hit was damaged badly and to complicate the matter, the car is owned by a barrister who also has a military background. Imagine how she will successfully explain to him that her intentions were not exactly what her actions suggested.
This is the reason I normally tell people that "an action may not always fall into place with intentions". This is why we should also try to look beyond what the actions represent and focus on what the intentions may suggest. Always allow people the chance to speak about their intentions and don't just assume based on their actions. Remember this, your assumptions are not failure-proof.
