I've put together a letter for your local GP's. It's a "notice of liability" in regards to the vaccine roll-out.

Largely aimed at UK citizens (but of course edit to suit your location), this can be handed in when visiting a general practice or medical establishment where they are carrying out the delivery of vaccinations. Ideally print multiple copies so they can be read by each GP at the practice. Personally I'll be trying to distribute these to as many practices as possible.
