Hoy estuve leyendo sobre este síndrome y me pareció interesante compartirlo con ustedes, pues como muchas veces se presenta, lo dejamos pasar y si hablo de un dolor de hombro una patología muy común normalmente se relaciona con el maguito bajito rotador (grupo de músculos y tendones que sujetan la articulación del hombro y nos permiten mover el brazo y el hombro). Por lo que puede tratarse de una contracción muscular que produce dolor.
Today I was reading about this syndrome and I thought it was interesting to share it with you, because as it often occurs, we let it pass and if I talk about a shoulder pain a very common pathology is usually related to the rotator cuff (group of muscles and tendons that hold the shoulder joint and allow us to move the arm and shoulder). So it may be a muscle contraction that causes pain.
Lo curioso es que, si la persona presenta dolor de hombro, contracción de la pupila y el parpado caído automáticamente deberíamos realizarnos un chequeo por que podríamos estar presentando un Tumor de Pancoast, ápice del pulmón no es muy común y a veces suele diagnosticarse muy tarde. Fue descubierto por el radiólogo estadounidense Dr. Henry Pancoast por eso se llama asi, suele presentarse en hombres que fuman de edades comprendidas entre los 40 y 60 años, los síntomas que presentan son:

síndrome de horner
edema del brazo (por que se comprimen los vasos sanguíneos)

The curious thing is that if the person presents shoulder pain, pupil contraction and eyelid drooping, we should automatically have a checkup because we could be presenting a Pancoast tumor, apex of the lung is not very common and sometimes it is usually diagnosed very late. It was discovered by the American radiologist Dr. Henry Pancoast that is why it is called so, usually occurs in men who smoke between the ages of 40 and 60 years, the symptoms are:
atrophy of the hand and arm musculature
horner's syndrome
oedema of the arm (because the blood vessels are compressed).

Normalmente el cáncer de pulmón presenta dolor constante, tos con sangre, dificultad para respirar entre otros, pero el tumor de Pancoast no presenta esta sintomatología como nombramos anteriormente. Los síntomas del tumor de Pancoast se dan por que este interfiere entre los nervios y vasos sanguíneos que conectan con el brazo, en la imagen a continuación veremos más características para que conozcas.
Normally lung cancer presents constant pain, coughing up blood, difficulty breathing among others, but the Pancoast tumor does not present this symptomatology as mentioned above. The symptoms of Pancoast tumor occur because it interferes between the nerves and blood vessels that connect with the arm, in the image below we will see more features for you to know.
Para finalizar puedo decir que si tenemos un paciente con dolor de hombro debemos preguntarle si alguna vez han tenido cáncer, si son fumadores o presentan alguno de los síntomas es decir una pupila as contraída (horner), un parpado caído o falta de sudoración en esa zona del rostro, para así realizar las pruebas pertinentes y descartar un tumor de Pancoast. Finally I can say that if we have a patient with shoulder pain we should ask them if they have ever had cancer, if they are smokers or if they have any of the symptoms i.e. a contracted pupil (horner), a drooping eyelid or lack of sweating in that area of the face, in order to perform the appropriate tests and rule out a Pancoast tumor.
