Uno de los procedimientos que realiza enfermería a diario en cualquier servicio del área de salud es el examen físico, este es indispensable para la detección de alteraciones que puedan comprometer su salud.
One of the procedures that nursing performs daily in any service in the health area is the physical examination, this is essential for the detection of alterations that may compromise your health.
El examen físico es un procedimiento de enfermería que consiste en realizar una valoración completa del paciente, a través de la observación acompañada de una serie de técnicas que permiten determinar si hay alguna alteración en el estado de salud, a través de él se puede confirmar los datos que el paciente manifestó.
The physical examination is a nursing procedure that consists of carrying out a complete assessment of the patient, through observation accompanied by a series of techniques that allow to determine if there is any alteration in the state of health, through it, the data that the patient reported.
El examen físico puede ser de tres formas
The physical examination can be in three ways
Cefalocaudal: es el más común, requiere de una valoración detallada y se realiza desde la cabeza hasta los pies.
Cephalocaudal: it is the most common, requires a detailed assessment and is performed from head to toe.
Por regiones: en este solo se valora la parte afectada, se utiliza en casos de emergencia donde se requiera de actuación rápida.
By regions: in this only the affected part is valued, it is used in emergency cases where rapid action is required.
Por sistemas: este se realiza con el fin de valorar cada sistema o aparato de forma detallada, cada uno se valora de una forma distinta por ejemplo: se comienza desde el sistema, respiratorio, cardiovascular, digestivo, genitourinario , hemolinfopoyetico, osteomioarticular, y por último el sistema nervioso.
By systems: this is carried out in order to assess each system or device in detail, each one is assessed in a different way, for example: it starts from the respiratory, cardiovascular, digestive, genitourinary, hemolymphopoietic, osteomyoarticular, and by last the nervous system.
El objetivo del examen físico es que posterior a la valoración física se realice un diagnóstico, formar un plan de cuidados de acuerdo a las necesidades del paciente para así ayudar a restablecer la salud.
The objective of the physical examination is that after the physical evaluation a diagnosis is made, forming a care plan according to the patient's needs in order to help restore health.
Inspección: este se realiza a través de la observación.
Inspection: this is done through observation.
Palpación: se realiza a través del sentido del tacto, con el fin de determinar las características como: forma, temperatura, posición, textura, presencia de dolor.
Palpation: it is done through the sense of touch, in order to determine the characteristics such as: shape, temperature, position, texture, presence of pain.
Percusión: este se realiza a través del tacto y la audición, consiste en percutir con la yema de los dedos la estructura corporal que se quiere explorar.
Percussion: this is done through touch and hearing, it consists of percussion with the fingertips the body structure that you want to explore.

Auscultación: se realiza a través de la audición, se puede realizar de forma indirecta colocando el oído sobre la región que se quiere escuchar, y de forma directa utilizando el estetoscopio.
Auscultation: it is done through hearing, it can be done indirectly by placing the ear over the region to be heard, and directly using the stethoscope.
Realizar el lavado de manos. explicarle al paciente el procedimiento que se va a realizar, conservar su privacidad, el paciente debe estar en una posición cómoda, mantener un ambiente iluminado.
Carry out hand washing. explain to the patient the procedure to be performed, preserve their privacy, the patient must be in a comfortable position, maintain a bright environment.
Antes de comenzar debemos observar la apariencia del paciente, como por ejemplo: la postura, la higiene personal, los movimientos corporales, el lenguaje y tono de voz, la marcha, peso, estatura, coloración de la piel.
Before starting we must observe the appearance of the patient, such as: posture, personal hygiene, body movements, language and tone of voice, gait, weight, height, skin color.
Como su nombre lo indica comenzamos por la cabeza: evaluamos la forma, tamaño, posición y palpamos en busca de protuberancias, también observamos el cabello, palpamos para saber su textura, cantidad, si hay presencia de caspa, o seborrea.
As its name implies, we start with the head: we evaluate the shape, size, position and feel for bumps, we also observe the hair, we feel for its texture, quantity, if there is presence of dandruff, or seborrhea.
En la cara: observamos facies, simetría, color, si hay presencia de cicatrices y manchas, en los ojos observamos las pupilas y movimientos oculares.
On the face: we observe facies, symmetry, color, if there are scars and spots, in the eyes we observe the pupils and eye movements.

Oídos: se debe observar si hay presencia de cerumen o secreción, así como también el tamaño.
Ears: it should be observed if there is presence of earwax or discharge, as well as the size.
En la nariz: inspeccionamos la simetría, forma, si ambas fosas nasales están permeables o si hay sangrado nasal (epistaxis)
In the nose: we inspect the symmetry, shape, if both nostrils are permeable or if there is nasal bleeding (epistaxis).

Boca: observamos la hidratación de las mucosas, si hay presencia de halitosis, dentadura completa o no.
Mouth: we observe the hydration of the mucous membranes, if there is presence of halitosis, complete teeth or not.

Cuello: observamos la movilidad del mismo, palpamos el pulso carotídeo.
Neck: we observe the mobility of the neck, we palpate the carotid pulse.

Cavidad torácica: observamos simetría, forma, edema, en la espalda; si hay presencia de desviaciones en la columna vertebral. Por otra parte se debe de realizar la auscultación tanto pulmonar como cardíaca utilizando un estetoscopio.
Thoracic cavity: we observe symmetry, shape, edema, in the back: if there is presence of deviations in the spine. On the other hand, both pulmonary and cardiac auscultation should be performed using a stethoscope.

Abdomen: se observa si hay estrías, cicatrices, distensión abdominal, se debe palpar en busca de protuberancias salientes así como también se debe auscultar con un estetoscopio para la valoración de los ruidos hidroaéreos.
It is observed if there are stretch marks, scars, abdominal distention, it should be palpated in search of protruding lumps as well as it should also be auscultated with a stethoscope to assess air-fluid sounds.

Genitales: para esta valoración es importante cuidar la privacidad del paciente, en la mujer se debe observar los labios mayores y menores, si hay presencia de lesiones o dolor, en el hombre explorar pene, testículos, escroto, en ambos se debe palpar el pulso femoral.
Genitals: for this assessment it is important to take care of the privacy of the patient, in women the labia majora and minora should be observed, if there is presence of lesions or pain, in men explore the penis, testicles, scrotum, in both the pulse should be palpated femoral.
Extremidades
Extremities
Superiores: se debe valorar la integridad cutánea, explorar si hay presencia de edema, rigidez, tono muscular, fuerza, y palpar él pulso cubital.
Upper: skin integrity should be assessed, examined for the presence of edema, rigidity, muscle tone, strength, and the ulnar pulse palpated.
Inferiores: determinar la simetría, si hay presencia de dolor, rigidez, tono muscular, edema, flexión y extensión.
Lower: determine the symmetry, if there is presence of pain, stiffness, muscle tone, edema, flexion and extension.

También es importante realizar una valoración del estado mental donde se podrá determinar el nivel de conciencia del paciente, la memoria, el habla, el lenguaje, y si está orientado o no.
It is also important to carry out an assessment of the mental state where the patient's level of consciousness, memory, speech, language, and if he is oriented or not can be determined.
El examen físico cefalocaudal requiere de mucha paciencia, ya que de esta forma obtendremos los resultados adecuados.
The cephalocaudal physical examination requires a lot of patience, since in this way we will obtain the appropriate results.
