It looks like it was damaged in some sort of hydraulic press, or maybe the Quarter was used to jack up a car or something. The edge looks like it was filed down for some reason.

It's difficult to know what exactly happened, it might be intentional, but more likely two quarters got stuck side-by-side in a vending machine and one got bent. That explains the filing of the raised portion. Not sure, but that's my best guess.
A little disappointed it's not an error, but I can see why it's not.
