You will start as Miles Upshur, a journalist with the desire to write the story of the decade, but instead you will live the worst nightmare of your life... Comenzaras como Miles Upshur, un periodista con ganas de escribir la historia de la década, pero en vez de eso va a vivir la peor pesadilla de su vida... El concepto de este survival horror es bastante simple, a diferencia de otros, aquí no cuentas con armas ni con defensa propia como tal, el personaje lejos de ser capaz de atacar como cualquiera lo haría, con puños, armas, un palo o con lo primero que encuentre, es más enfocado en captar y grabar todos los sucesos ocurridos dentro del complejo, esto te doblega a que, para sobrevivir dentro de este lugar, tendrás que esconderte, ser bastante astuto y sigiloso para poder escapar de este infierno.

Para desplazarte dentro del asilo, cuentas como antes te mencione, con una cámara de vídeo, la cual es vital para poder avanzar y encontrar la salida, ya que gran parte del recinto está completamente a oscuras y dependes de la visión nocturna que tiene la cámara para guiarte y detectar a los perseguidores o los “Variantes” rezagados, los cuales pueden atentar contra tu vida al intentar capturarte.

También conocido como Billy, es el preso más popular dentro del recinto y el que probablemente inicio toda la masacre en el asilo, te toparas con él varias veces al principio el cual actuara inofensivo, pero después a la mitad del juego comenzará a atacarte y será uno de los persecutores más difíciles de evadir, ya que se manifiesta como un espectro o fantasma, el cual solo podrás ver cuando este lo suficientemente cerca, y con la visión nocturna activada, todo esto debido a que la corporación Murkoff lo estuvo utilizando como principal conejo de indias para sus experimentos, llevando todo al extremo y transformándolo en el espectro más despiadado del juego, también es el que te quitara de encima al intenso de Chirs.

No solo eso, también te acompañaran unas altas dosis gore, el cual nuestro héroe reacciona de maneras que muchos de nosotros haríamos, este nivel de nerviosismo y reacción hace que puedas sumergirte en el terror que siente el personaje, aparte de esto, muchos de los pobres desgraciados que pueblan el asilo también realizan sonidos, movimientos y monólogos que te darán una experiencia bastante aterradora y abrumadora.
