Hello to the whole community, today I received an emergency call, from a patient who suffered a traumatic fall from a motorbike and fractured a fixed crown held by a natural stump leaving the pulp exposed.
Se trata de un paciente masculino de
49 años, al llegar a la consulta manifiesta mucho dolor, el diente fracturado es el 21 (incisivo central superior izquierdo), al examen clínico se obseva la raíz con la pulpa expuesta, inflamada y con hiperemia.
This is a male patient from 49 years old, on arriving at the consultation he shows a lot of pain, the fractured tooth is 21 (central left upper incisor), on clinical examination the root is observed with exposed pulp, inflamed and with hyperemia.
El plan de tratamiento es el siguiente, realizar la primera sesión de endodoncia, utilizar la corona que se fracturó como provisional por lo tanto debemos desgastar el muñón natural que quedó dentro de la corona y colocar un perno provisional y cementarlo a la corona, y fijarla con un cemento temporal, para poder continuar la endodoncia.
The treatment plan is as follows: perform the first endodontic session, use the crown that was fractured as a provisional one, therefore we must wear out the natural stump that was left inside the crown and place a provisional pin and cement it to the crown, and fix it with a temporary cement, to be able to continue the endodontics.
Primero procedemos ha colocar anestesia local infiltrativa e intrapulpar, para poder realizar la extirpación del la pulpa izquémica, se lava y comenzamos la preparación biomecánica con una medida tentativa.
First we proceed to place local infiltrative and intrapulpatory anesthesia, in order to perform the removal of the left pulp, it is washed and we start the biomechanical preparation with a tentative measure.
Al tener el conducto preparado, medimos el pin provisional, debe tener al menos 10mm, tomanos la corona y desgastamos el interior para utilizarla como corona temporal.
When we have the duct prepared, we measure the provisional pin, it must have at least 10mm, we take the crown and we wear out the inside to use it as a temporary crown.
Luego, que logramos adaptar la corona al pin, mezclamos acrílico y primero en el pin para que de ajuste dentro del conducto y no se balacee, y luego colocamos acrílico en la corona y los unimos de manera que el pin y la corona sean uno, luego que el acrílico endurezca, se procede a fijarla con cemento temporal.
Then, that we manage to adapt the crown to the pin, we mix acrylic and first in the pin so that it fits inside the duct and does not bulge, and then we place acrylic on the crown and join them so that the pin and the crown are one, after the acrylic hardens, we proceed to fix it with temporary cement.
Esto es una solución provisional por la urgencia del dolor, pero el paciente debe venir a una próxima cita para culminar la endodoncia, y seguir utilizando la corona como provisional hasta que los laboratorios comienzen a laborar y podamos realizar un perno definitivo.
This is a provisional solution due to the urgency of the pain, but the patient must come to a next appointment to finish the endodontics, and continue using the crown as provisional until the laboratories start working and we can perform a definitive pin.
Se le recomienda al paciente no morder con ese diente alimentos duros ni muy tostados debemos evitar que la raíz se fracture y eso seria un gran fracaso.
It is recommended that the patient does not bite hard or very toasted foods with that tooth, we must avoid the root to fracture and that would be a great failure.
