La película trata de resaltar la problemática que existe en la India sobre el sistema educativo. Los tres idiotas, son tres jóvenes que ingresan en una universidad prestigiosa, se hacen muy buenos amigos, y se solidarizan entre ellos para poder aguantar el terrible sistema educativo, en la carrera de ingeniería, que se basa en la competencia entre cada estudiante por ser el mejor. Sin embargo estos tres personajes logran una muy buena amistad, basada en valores que se contraponen a la competencia que existía en la universidad entre los estudiantes.
Los padres de las familias hindúes, sueñan con que sus hijos sean ingenieros, muchos son obligados a estudiar esa carrera y no lo que les gusta, en eso se basa la trama principal de la película, muchos estudiantes de esa universidad se suicidaron por la presión que ejercen en ese sistema educativo.

Rancho es un personaje especial, es muy inteligente y creativo. Farhan y Raju no suelen sacar buenas notas, ellos sueñan con estudiar lo que les gusta, pero son obligados por sus padres, y la presión de no lograr el índice académico, hace que Raju casi se suicide también. Rancho, no siempre entraba a clases, pero era muy inteligente, y no le gustaba aprender de memoria los conocimientos.
El director Viru (Virus) ejercía una presión terrible sobre los estudiantes, destruía toda esperanza en los alumnos, se daba cuenta de la inteligencia de Rancho, pero no la aceptaba, y siempre estaba buscando hacerlo caer. Era la pesadilla de todos los estudiantes, su hijo también se suicidó por la presión del régimen de la carrera de ingeniería, pero a pesar de eso, Viru no cambió.
Buscaba destruir a Rancho, Farhan y Raju. Farhan y Raju lograron vencer los miedos y afrontar a sus padres y explicarles que no querían estudiar eso, sino seguir sus ideales. En una de las escenas casi finales, Viru intentó enredar a los “tres idiotas” en el hecho del robo de un examen final, ellos se van de la universidad en medio de la lluvia y se consiguen con que una de las hijas de Virus se le presentó el momento del parto, por la falta de electricidad debido a la lluvia, terminaron ellos tres ayudándola, demostrando su inteligencia, inventaron una máquina para que pudiera nacer el bebé. Gracias a esto, Viru, tuvo que dejar su soberbia a un lado y entender que la inteligencia no está en la calificación de un examen o una materia.
Después de todo esto Rancho desaparece por 10 años, y esta historia finaliza con sus amigos buscándolo, logran localizarlo gracias a un compañero (Chantur) de la universidad que los llamaba “idiotas”, y se burlaba mucho de ellos. Rancho creó una comunidad a disposición de niños abandonados. Los niños creaban cosas creativas, todo esto los ayudaba a su desarrollo. Chantur los creía “idiotas”, pero sería idiota considerarlos así. Muchas veces calificamos a las personas así porque son diferentes a nosotros, porque no juzgan por las apariencias, ni viven de las competencias, ni luchando por las excelencias sociales, si no que buscan el mejor sentido de la vida y la disfrutan al máximo, buscando la verdadera felicidad desde los pequeños detalles. Idiotas serían aquellos que se les va la vida en competencias y apariencias. Hagamos en la vida lo que nos gusta hacer, según nuestros talentos y cualidades, y no lo que el mundo quiere obligarnos hacer, si ser “idiota” es ser feliz, entonces seamos verdaderamente idiotas en la vida.
