The period leading up to the album's release was fraught with personal challenges for Osbourne. After the conclusion of the Bark at the Moon tour, he checked himself into the Betty Ford Clinic to combat his battles with alcoholism and drug addiction. During Osbourne's rehabilitation, a group of musicians, including bassist Bob Daisley, guitarist Jake E. Lee, and drummer Tommy Aldridge, began writing songs for The Ultimate Sin in Palm Springs. Osbourne briefly participated via day passes from rehab, but the bulk of the writing developed in his absence.
