I'd rather not have a party or be the center of attention on my birthday.

My anxiety was spiked by the surprise.

No surprise parties for me. If I'm not happy to see someone, you will know. So if I entered a room and heard "SURPRISE", then, the bigger surprise would be my reaction. I probably would just close the door and walk away, no fuss... That would be my birthday totally ruined, and perhaps the "friendship(s)" of those who surprised me:)
