I personally like to leave my camera in the camera bag for a while before I get it out and upload the photos to the computer. Really, you should wait until it reaches room temperature, which can take a few hours. Realistically, I usually wait about an hour, depending on how cold it was outside and how long the camera was exposed to that temperature. It's not an exact science for me, I usually touch the camera after about an hour of being inside and if feels cool, but not chilly cold, I will take it out and start the upload.
I rarely change lenses when out in the cold, and I never change lenses while the warm-up process is happening. I also use a camera that is very weather resistant and has lots of rubber seals (The Canon 5DS R). Weather resistant L series lenses also help me. Since I am mostly a landscape photographer, I find it wise to spend the extra money on weather resistant gear.
