Los abscesos periapicales agudos son muy comunes, generalmente los pacientes cuando lo presentan recurren a la extracción dental, pero no obstante pueden tratarse con las endondoncias, por ende es muy importante tratar las caries justamente a tiempo evitando que puedan progresar ya que cabe destar que una vez que el proceso infeccioso este totalmente instaurado en el apice del diente debemos tomar antibioticos y analgésicos, sino se logra tratar la infección al momento de realizar una exodoncia de la pieza dentaria que presenta el absceso, el anestésico local no tendrá su efecto, debido a que el ph esta ácido, por lo que causará gran dolor en el paciente, en esta publicación les hablaré del absceso periapical agudo para que podamos ver evitarlo y así evitemos momentos totalmente desagradables en el odontólogo.
Acute periapical abscesses are very common, generally patients when they present them resort to dental extraction, but nevertheless they can be treated with endondontics, therefore it is very important to treat caries just in time to prevent them from progressing, as it is worth noting that once the infectious process is fully established in the apex of the tooth we must take antibiotics and analgesics, if the infection is not treated at the time of exodontia of the tooth with the abscess, the local anaesthetic will not have its effect, due to the fact that the pH is acidic, which is why it will cause great pain in the patient. El absceso periapical aguda es la inflamación causada por la acumulación de agentes bacterianos, se origina justamente en el ápice del diente este es la consecuencia de caries no tratadas, donde tambiem se engloban los tejidos que se encuentran alrededor del diente, evoluciona de una manera muy rápida y ocasiona fuerte dolor. La causa de estas infecciones son los diversos agentes físicos, microbianos y químicos, normalmente conservando estrecha relación con las patologías inflamatorias reversibles e irreversibles del organo pulpar de la estructura dental, al estar la menciona pulpa involucrada este absceso se desarrolla por vía de la bolsa periodontal.
Acute periapical abscess is the inflammation caused by the accumulation of bacterial agents, it originates precisely in the apex of the tooth and is the consequence of untreated caries, where the tissues around the tooth are also involved, it evolves very quickly and causes severe pain. The cause of these infections are various physical, microbial and chemical agents, normally closely related to reversible and irreversible inflammatory pathologies of the pulp organ of the tooth structure. El tratamiento para el absceso periapical agudo consite en la medicación con antibióticos normalmente la amoxicilina +Acido Clavulonico, o las penicilinas naturales, una vez que la medicación ya este totalmente cumplicada por el paciente se debe prodecer al drenaje del absceso apical agudo, este se realiza mediante la vía del ligamento periodontal, así permitiendo la salida del exudado purulento. En algunos casos se indica el tratamiento de conducto o pulpectomia, eliminando completamente la pulpa vital. Nunca se debe drenar un absceso periapical si el pacientes no ha tomado antibiotico.
The treatment for acute periapical abscess consists of antibiotic medication, normally amoxicillin + Clavulanic acid, or natural penicillins. Once the medication has been fully complied with by the patient, the acute apical abscess must be drained, which is carried out via the periodontal ligament, thus allowing the purulent exudate to drain. In some cases, root canal treatment or pulpectomy is indicated, completely removing the vital pulp. A periapical abscess should never be drained if the patient has not taken antibiotics.
En cuanto a los sintomas el paciente presenta odontología normalmente como un dolor que punsatil que se puede extender desde la mandibula, hasta el oido, así como también molestia o dolor a los cambios de temperatura fríos y calientes, dolor al masticar o morder, el paciente puede manifestar fiebre alta así como cambios de temperatura, como rubor y calor en las mejillas, presentando dificultad para tragar y masticar, adenopatias en los ganglios del cuello, halitosis o mal aliento, trayecto fistulo o exudado con olor y sabor desagradable.

Por último las imágenes que pudimos observar en esta publicación corresponden al absceso periapical agudo de una paciente que presentaba, caries profunda no tratada que comprometio a la pulpa ocasionando inflamación y formación de la secreción purulenta en el apice del diente, la paciente presento gran dolor durante la extracción dental debido a que seguía complentamente instaurado el absceso periapical, no olviden en tratar sus caries dentales justo a tiempo para evitar que esto ocurra y perder una pieza dentaria.
Finally, the images that we were able to observe in this publication correspond to the acute periapical abscess of a patient who presented untreated deep caries that compromised the pulp causing inflammation and formation of purulent secretion in the apex of the tooth, the patient presented great pain during dental extraction because the periapical abscess was still fully established, do not forget to treat your dental caries just in time to prevent this from happening and to avoid losing a tooth.
