El queratoquiste odontogénico, es una neoplasia cuando hablamos de neoplasia nos referimos a una proliferación anormal de tejido que forma un tumor puede ser maligno o benigno, por lo tanto está patología bucal forma parte de las neoplasias odontogenicas, recibe por nombre Tumor Odontogénico Quístico Queratinizante, desde hace algunos años se dejó de considerar quiste y pasó a ser un tumor, debido a su conducta, ya que crece de una manera muy rápida y causa mucha destrucción en los tejidos, por lo que para retirarlo se debe mutilar una parte del hueso afectado, y en una conducta agresiva.
The odontogenic keratocyst is a neoplasm when we talk about neoplasm we refer to an abnormal proliferation of tissue that forms a tumour, it can be malignant or benign, therefore this oral pathology is part of the odontogenic neoplasms, it is called Keratinising Cystic Odontogenic Tumour, For some years now it has ceased to be considered a cyst and has become a tumour, due to its behaviour, as it grows very quickly and causes a lot of destruction to the tissues, so that to remove it a part of the affected bone must be mutilated, and in an aggressive behaviour.
En cuanto a su localización con mayor frecuencia será encontrado en la mandíbula específicamente en el sector posterior en la zona de los molares y también en la rama ascendente, este tiende estar asociado a un diente incluido es decir que no logra su erupción, se origina por restos de la lámina dental.
As for its location, it is most frequently found in the mandible, specifically in the posterior sector in the area of the molars and also in the ascending branch; it tends to be associated with an included tooth, i.e. one that has not erupted, and originates from the remains of the dental lamina.
Lo encontraremos en pacientes de 10 a 40 años más frecuente en los hombres generalmente no genera ninguna sintomatología solo ocasionará dolor si este produce una infección, tiende a tener alta recidiva, puesto que vuelve a formarse una vez una vez que se elimina, su crecimiento se dará de una manera antero posterior es decir desde adelante hacia atrás.
It is found in patients between 10 and 40 years of age and is more common in men. It generally does not generate any symptoms, it only causes pain if it causes an infection, it tends to have a high rate of recurrence, as it will form again once it has been removed, its growth will be anteroposterior, i.e. from the front to the back.
Uniloculares pero a medida que van creciendo se torna multiloculares, radiograficamente lo vamos a observar como una imagen radiolúcida unilocular y multilocular qué tiene una estrecha relación a un diente incluido, pero esta cubierto de epitelio, este tiende a ser muy versátil se puede aislar en las ramas pero también esto se puede ubicar entre las raíces de los dientes, asociado a la corona a la raíz.
Unilocular but as they grow they become multilocular, radiographically we will observe it as a unilocular and multilocular radiolucent image which has a close relationship to an included tooth, but it is covered with epithelium, this tends to be very versatile, it can be isolated in the branches but it can also be located between the roots of the teeth, associated with the crown to the root.
El paciente puede tener mas de un queratoquiste al mismo tiempo, puede ser periférico y formarse afuera del hueso como consecuencia de la lamina dental, el pronóstico que posee el queratoquiste es reservado no es favorable porque tiene alta recidiva, puesto que se puede transformar en maligno pero es menos frecuente que el quiste dentigero.
The patient may have more than one keratocyst at the same time, it may be peripheral and form outside the bone as a consequence of the dental lamina, the prognosis of the keratocyst is not favourable because it has a high recurrence, as it can become malignant, but it is less frequent than the dentigerous cyst.
Sus característica clínicas esta va infiltrar en el hueso, pero no ocasiona asimetría facial a pesar del gran crecimiento que posee, pero si se expande por el hueso, radiograficamente también se asemeja a una flor festoneada bien definidas, que se encontrarán alrededor del diente.
Its clinical characteristics are that it infiltrates the bone, but does not cause facial asymmetry despite its large growth, but it does expand into the bone, radiographically it also resembles a well-defined scalloped flower, which will be found around the tooth.
Su tratamiento debe ser netamente quirúrgico por medio de la marsupialización, cuando es muy grande, se debe tener higiene de una manera constante y con mucho cuidado puesto que genera muchos procesos infecciosos, la exéresis, es decir el corte generalmente es agresivo pero primeramente debemos tomar una biopsia, para luego establecer el diagnóstico una vez que se tenga se debe eliminar por completo, la lesión debemos eliminar el hueso y el tejido vecino puesto que tiene un alto riesgo de recidiva.
Its treatment must be purely surgical by means of marsupialisation, when it is very large, hygiene must be maintained constantly and with great care as it generates many infectious processes, exeresis, i.e. the cut is generally aggressive but first we must take a biopsy, to then establish the diagnosis once the lesion is completely eliminated, we must eliminate the bone and the neighbouring tissue as it has a high risk of recurrence.
