An important point of differentiation is between selfishness and narcissism. Egotism or selfishness, in moderation, can be healthy and necessary for self-care. For example, putting oneself first occasionally, advocating for personal needs, or setting boundaries is not only healthy but necessary.
In contrast, narcissism—a clinical pathology—is characterized by an inflated sense of self-importance, lack of empathy, and a demand for excessive admiration. Unlike healthy selfishness, narcissism is a destructive, pathological state that often results in emotional harm to others.
