Millitary history is full of examples of major blunders being caused by survivorship bias and the idea that what worked before would certainly work again. Like carnage in US Civil War being caused by commanders of both sides using Napoleonic era tactics based on tight formations and massive infantry charges or French before WW2 believing that they would stop Germans if they simply build massive and seemingly impenetrable line of field fortifications, like the trenches of WW1.
