PRETERM PRELABOUR RUPTURE OF MEMBRANE

PPROM
Definition: PPROM means preterm premature rupture of membrane , it means that your waters have broken earlier than normal I.e before labour starts. It is more common in preterm deliveries.

Causes: In most cases, the cause is not known. But it has been linked with an infection in the vagina caused by a bacteria ( BV).

Effects of PPROM:

o Preterm delivery
o Infection in the womb ( chorioamnionitis)
o Risk of foetal death
o Offensive vaginal discharge

Investigations:

• Blood test
• Vaginal swab
• Urine test
• Ultrasound

Management:

On presentation to the hospital, if the diagnosis of PPROM is confirmed, you will be admitted into the hospital for about 48 hours; If you do not go into labour within this time interval, you will be managed at home.

When to call for help if managed at home:

• Raised temperature (more than 37ºC)
• Vaginal bleeding
• Leaking fluid becomes greenish or smelly
• Abdominal pain
• Contractions

Treatment:

The purpose of the treatment is to prevent infection and help get ready for birth.
o Antibiotics
o Steroids
o Delivery when appropriate
